Enrollment & Billing

$299 one-time enrollment fee, which is non-refundable.
$99 monthly membership. The first $99 payment is due 30 days after enrollment. Authorized Pure Radiance staff configure membership payments and recurring billing directly in Boulevard.
Membership enrollment and monthly payments are paid directly. Cherry, Affirm and Klarna are separate from membership payments and may be available for eligible treatments outside the membership.

The membership has an initial 3-month commitment and continues month-to-month afterward.
After the initial commitment is completed, cancellation requires 30 days’ written notice, subject to the approved membership agreement.

The first 30 units are available upon enrollment, subject to provider evaluation and approval.
30 units become available every 90 days while membership remains active and current. Unused units expire at the end of each 90-day period and do not roll over.
Additional Botox units are $9 per unit when provider evaluation determines they are clinically appropriate.
Membership does not guarantee that all 30 units will be administered during every treatment. Treatment and dosage remain subject to provider evaluation and clinical appropriateness.

Members receive 10% off eligible services and products.
Exclusions include EndoLift, chin liposuction, weight-loss programs and medications, memberships, packages, already discounted services, promotional pricing, special events, financing-related purchases and certain retail products.
Membership savings cannot be combined or stacked with other discounts or promotional offers. When a promotional price provides greater savings, the member may elect to use the promotional price instead.

A complimentary DiamondGlow treatment may be redeemed during the member's birthday month while membership is active and in good standing. It expires at the end of the birthday month and is non-transferable.
Member access may include early event invitations, member-only offers, skincare education and tips, early access to select promotions, and occasional VIP or member-only opportunities. No recurring schedule or frequency is promised.

Membership and all benefits are for the enrolled member only. Botox units, discounts, the birthday DiamondGlow and other benefits cannot be transferred, shared or gifted.
Benefits have no cash value and cannot be exchanged for cash or account credit.
Unused benefits expire when membership is canceled or terminated.

Temporary pauses may be approved by Pure Radiance for qualifying circumstances such as pregnancy, qualifying medical circumstances or extended travel.
Pause duration is determined individually based on member circumstances and Pure Radiance approval. Membership benefits are suspended during an approved pause.

Membership must remain active and current for benefits to be used.
If payment fails or an outstanding membership balance exists, benefits are suspended and cannot be redeemed. Benefits resume after the account is brought current.
Appointments are subject to availability. Standard appointment cancellation and no-show policies apply.

Existing Pure Radiance Botox patients may enroll at any time; there is no specific enrollment period.
Treatment remains subject to provider evaluation and approval.
